Patient Registration Form 

 

Are you a new patient ? Or an existing patient but haven't been seen for over 2 years ?

By filling in a patient registration prior to your appointment, it will help save time for both yourself and our staff by updating your details before your appointment.

 

The forms can be returned via email rockhamptoneyeclinic@mercycq.com or at the time of your appointment.  

 

 

 

 
 
General Appointment Information
 

Referral Policy

All patients must be referred by their General Practitioner, Optometrist or Specialist. Referrals will remain current for 12 months (from your appointment date) from a General Practitioner and Optometrist or 3 months (from your appointment date) from a Specialist.

 

Appointments

Consultation time may vary and can take anywhere from 90 minutes to 120 minutes (depending on the type of tests conducted).  Unfortunately sometimes we do run late. On the day of consultation, if you can’t wait longer, you can approach the reception for re-scheduling.

 

Your eyes may be dilated at your appointment so please be aware you will need a driver to take you home after your appointment as you will not be able to drive.

 

Please bring a pair of sunglasses to wear after your appointment.

 

Time is wasted each week due to patients not advising us in advance that they will not be attending their appointment. These are valuable appointments that could be given to someone else. We ask that you contact the clinic as soon as you receive your appointment to confirm you will attend. 

 

If you feel your condition is deteriorating or you feel you need to be seen sooner than you’re allocated appointment, please contact your referring GP or Optometrist.

 

Please note that we are NOT a Bulk Billing Practice and we require payment IN FULL on the day of your appointment.  This includes payment for your consultation and any extra investigations.

 

Treating Practice Staff with Respect

We value all of our staff who work very hard to provide you with a first class service. Please treat them with the same respect you would expect yourself.

 

Smoke-Free Buildings

The Mater operates a smoke-free policy. Smoking is not allowed in any part of the practice premises
